Did you miss MYSO Registration?

Openings still exist in all age groups but space is filling up quickly! 
(As age groups fill, we will post that information on the web site.) 

What should you do now if you still want to register?

1)  Print the registration form and waiver located under the documents tab. 

2)  Fill it out and mail it to MYSO, PO Box 182, Morton, IL 61550. 
Late registration will only be accepted by mail.

3)  Be sure to include your check, payable to MYSO. 
Late fees do apply now:
1 child, $65
2 children, $110
3 children, $130
4 or more children, $140

Reminder:  The guaranteed acceptance deadline for participation was May 14th.  All late registration is subject to availability and may not be accepted if an age group fills.  If your child's age group is full when your late registration is received, you will be contacted via phone or email.

MYSO is for all boys and girls in Kindergarten – 8^th grade.  9^th grade girls are also eligible to play.  (Grades are based on the grade your child will enter in the fall of 2012.)

Kindergarteners will participate in a weekly clinic, with an emphasis on developing fundamental skills and knowledge about the game of soccer. 

Teams will be divided into the following categories:
1^st/2^nd grade girls
1^st/2^nd grade boys
3^rd/4^th grade girls
3^rd/4^th grade boys
5^th/6^th grade girls 
5^th/6^th grade boys               
Co-ed teams compromised of 7^th/8^th/9^th grade girls and 7^th/8^th grade boys. 
 
Registration dates are Saturday, May 12 from 9:00 am to 11:00 am AND Monday, May 14 from 5:30 pm – 7:30 pm at the Morton Recreation Center, 324 Detroit Avenue

PLEASE DOWNLOAD THE REGISTRATION FORM AND WAIVER FROM THE MYSO WEBSITE AND FILL IT OUT PRIOR TO COMING TO REGISTRATION.  (Registration form is available under the documents tab.)

Register by mail.  Download the registration form from the MYSO website, fill it out, and mail it to MYSO, PO BOX 182, Morton, IL,  61550.  Registration must be postmarked by May 14, or it will be considered late and subject to late fees/team availability. 
 
2012 REGISTRATION ENDS May 14
May 14 is the guaranteed acceptance deadline for fall participation. 
ANY REGISTRATION RECEIVED AFTER MAY 14 IS CONSIDERED LATE AND MAY NOT BE ACCEPTED.
 
If you register your child AFTER May 14, there is no guarantee they will be allowed to play in the fall. LATE REGISTRATION IS SUBJECT TO AVAILABILITY IN EACH INDIVIDUAL AGE GROUP.
 
 
Registration Fees:
 
One Player                       $45.00                  
Two Players                      $90.00                  
Three Players                   $110.00
Four or more Players        $120.00 
 
If you register after May 14, please add a $20 late fee.  If you must drop for any reason, you must notify MYSO by July 1 in order to receive a refund.  After that date no refunds will be given.
